Update patient details

Overview

This flow describes how to update the details of a patient in Care. You edit the details from the patient's profile.

Pre-requisites

  • The patient is registered in Care.
  • You can open the patient's profile.
  • You have the permissions listed below.

Steps

1. Open the patient's profile

Open the profile of the patient that you want to update. See View a patient's profile.

2. Select Edit

Go to the "General Info" section or the "Additional Details" section. Select the Edit action on that section. Care opens the registration form. The form shows the current details of the patient.

3. Update the details

Change the details that you want to update. You can update these fields:

NameThe name of the patient.
GenderThe gender of the patient.
Phone NumberThe primary contact number of the patient.
Emergency Phone NumberThe contact number for emergencies.
Date of birth or AgeThe date of birth of the patient, or the age.
Blood GroupThe blood group of the patient.
AddressThe current address of the patient.
Permanent AddressThe permanent address of the patient.
PIN CodeThe postal code of the address.
Geo OrganizationThe administrative area of the patient.
Is DeceasedThe deceased status of the patient, with the date and time.

The form uses the same fields and the same validation rules as registration. For the details of each field, see Register a patient.

To record the death of a patient, see Mark a patient as deceased.

Note: If you leave the form with unsaved changes, Care shows a warning first.

4. Save the changes

Save the form. Care shows a success message. Care returns you to the patient's profile.

Expected Outcome

  • Care saves the new details of the patient.
  • Care shows a success message.
  • Care shows the patient's profile with the new details.
